Helpful Adoption Application Tips

  • Please make sure only one person in your household fills out an application. The person who fills out the application will be the person expected to come in and sign the contract for the pet if approved.

  • Please make sure you fill out all fields. Any areas outlined in blue or with blue text are staff only please leave those areas blank.

  • In order to process your application faster, we recommend attaching proof of current rabies and distemper vaccinations for any resident cat(s) and/or dogs.

Fill out and Submit an adoption application

  • Click the button to download the application to your computer.

  • Open w/Adobe Acrobat/Reader and fill out. You need to download Acrobat Reader. This will not work properly in a browser window or on a cell phone (required sections are outlined in red).

  • When you are done attach to an email and send to frontdesk@couleehumane.com.

  • We will process your application within 24 hours of receiving it.

  • If we’re not able to give your application a general approval or the animal you are interested in is no longer available we will reach out (typically by email) letting you know and going over what we still may need.

  • Adoption counselling is done over-the-phone between 1pm and 4pm each day. If your application was given a general approval and the animal you are interested in is still available, we will call within one to three business days to do that counselling.

  • Approvals for specific animals are only given once adoption counselling is done after which an appointment will be scheduled for you to sign adoption paperwork and officially take the animal home.

You can also print it and fill it in by hand as well. If you fill it out by hand you will need to bring it in to us for processing.

Pet Adoption FAQs

  • Since adoption entails a legally binding agreement, individuals interested in adoption must be at least 18 years old.

  • You should plan for 20 – 30 minutes to complete the adoption paperwork and pay the adoption fee.

    Remember to bring a collar and leash or a secure carrier for your new companion animal.

    Please make sure to bring your photo ID/driver’s license with you at the time of adoption.

    We strive to send home an adopted animal within the next 3-days after approval.

    A staff member will contact you within a few weeks, via phone or email, to see how you and your new companion animal are getting along, and to listen to your feedback about our adoption process!

  • All resident cats and dogs are required to have up-to-date rabies and distemper vaccinations before approval, unless your veterinarian provides documentation advising against it due to health or age concerns.

    Please provide the name of your veterinary care provider and the names of your current pets.

    Bringing proof of current vaccinations will expedite the adoption process, particularly on weekends when vet clinics are closed. Routine vaccinations are crucial for your pets' well-being and compliance with legal requirements.

    We do not approve applications for intact animals of the opposite sex within the same species, and we cannot approve the adoption of an intact animal to a household with an opposite-sex intact resident animal.

  • If you're not the homeowner, please ensure you've obtained your landlord's approval to have pets on the premises. Each landlord may have unique criteria regarding the presence of animals on their property.

  • Certainly! You would just need a Certified Veterinarian Inspection (CVI) prior to the adoption.
